RADIATION ONCOLOGY
The Radiation Oncology department at Subharti Cancer Center is equipped with state of the art equipment (TrueBeam STx for external beam radiotherapy and GammaMedplus iX for Brachytherapy). We have expert (Internationally trained-JCI Trained) Radiation Oncologists, Medical Physicist and Radiation Technologists. The sophisticated, state of the art equipment we use for delivering the best in cancer care are as follows.
TrueBeam STx: Most advanced medical linear accelerator designed for high precision (less than 1mm accuracy) treatment such as frameless Stereotactic Radiosurgery (SRS) and Stereotactic Body Radiotherapy (SBRT). It is capable of fast (2 minutes per day) and precise (Sub-millimeter accuracy) Radiotherapy.
With TrueBeam system, treatment can be performed with unparalleled ease, precision and speed. This technology make it possible to deliver fast, accurate image guided treatment within just a few minutes.
The linear accelerator is equipped with a beam shaper, On board imager (OBI-Kv), Orthogonal Kv X-ray imager, Cone Bean CT scanner (CBCT), 6D Robotic couch and Respiratory Gating (Real Time Position Management) system.
Treatment modalities offered:
Image Guided Radiotherapy (IGRT):
- • Radiation delivered after imaging the patient on treatment couch.
- • This ensure accurate treatment, addresses motion and positioning of the patient on the treatment couch and of tumor/organs that moves e.g. prostate, which may move consequent to the filling of the bladder or rectum that lie adjacent to it.
- • In addition it also provides information to the Radiation Oncologist regarding changes required in treatment parameters to account for the weight changes or response to treatment.
Frameless Stereotactic Radiosurgery (SRS):
- • It involves stereotactic guided delivery of very high radiation dose with sharply focused multiple radiation beams.
- • True Beam STx offers sophisticated treatment planning software and patient position system with sub-millimeter accuracy.
- • Unlike gamma knife SRS, Truebeam offers painless, knifeless SRS of malignant and benign brain lesions such as Glioma, Meningioma, AVM, Pituitary Adenoma, Acoustic Schwannoma, Trigeminal Neuralgia etc.
Stereotactic Body Radiotherapy (SBRT)/ Stereotactic Ablative Radiotherapy(SABR):
- • The principles of radiosurgery applied outside of brain, high dose radiation is administered in a sharply focused manner to destroy cancerous lesions.
- • This treatment is specifically designed for lesions and for patients with early stage lung cancer, suitable for surgery but unable to undergo the same because of medical reasons. Thus with this method localized lung tumor can be safely ablated without using anesthesia, surgical incision and prolonged hospital stay. Post SBRT patient’s quality of life is also better compare to surgery.
- • Now a days SBRT is being increasingly used as ablative procedure in treatment of Carcinoma Prostate, Carcinoma Pancreases, HCC, RCC and oligo-metastatic lesions to the lungs, liver and bone with controlled primary.
Intensity modulated radiotherapy (IMRT):
- • Application of computerized algorithms to create a radiation treatment that target the tumor and spares the normal tissue from radiation.
- • This treatment is standard of care in many disease sites e.g. brain, head and neck, lung, esophagus, stomach, liver, kidney and pelvic organ tumors.
- • It is applied in brain tumors to spare the visual pathway, brainstem and hippocampus which is the seat of memory.
- • In head and neck tumor to spare the salivary apparatus and the structure involves in swallowing.
- • Similarly in chest abdomen and pelvis tumor this technique used to spare the normal organs that lie adjacent to the tumor.
3D Conformal radiotherapy (3D-CRT):
3DCRT forms basis of modern radiotherapy techniques outlined above. The essence of this treatment method is the creation of a virtual model of each individual patient in a treatment planning computer and then creating a radiation plan using multiple radiation beams that match the shape of the tumor i.e. conform to the tumor.
Tumor Gating and Tracking:
- • TrueBeam STx at Subharti Cancer Hospital, offers a special andhighly precise radiation treatment modality which is normally not available to other cancer center.
- • This technique used in case where tumor keeps changing its position due to internal organ movement e.g. tumors of lung, breast, kidney and liver.
- • The system’s software track the tumor during phases of respiratory cycle and allow the machine to deliver the radiation once the tumor aligned in a particular window period. In this way we can treat the patient without causing much distress (breath hold, abdominal compression etc) and at the same time normal organs can also be spared from radiation exposure.
Deep Inspiratory Breath Hold (DIBH):At Subharti Cancer Hospital, we also provide DIBH technique of radiotherapy which is not available at other centers. This technique is specially used in treatment of left breast cancer.
Why DIBH ? The advancement of breast cancer treatment like early diagnosis and availability of new generation chemotherapy agents, targeted therapy and immunotherapy has prolonged the overall survival of breast cancer patient to many folds. In that case it is important to spare the heart from radiation exposure that in long term may cause coronary artery disease secondary to radiotherapy of left breast. Multiple studies have shown that DIBH technique significantly reduces the radiation dose to heart, thus reduces the risks of cardiovascular events in future and prolonged the patient’s life.
Advantages of TrueBeam STx:
- • True beam offers imaging tool that can verify not only the patient’s position, but also the magnitude of tumor motion during a treatment, enabling accurate tumor targeting.
- • Fast, automated treatment delivery software that significantly shortened the duration of sophisticated treatments involving different beam angles.
- • Rapid Arc radiotherapy technique, which delivers a precise image guided intensity modulated treatment in as little as two minutes.
- • Gated Rapid Arc capability make it possible to use this treatment approach for tumors of lung, kidney, breast and liver, when tumor motion is an issue.
High Dose Rate Brachytherapy (HDR):
- • Technique of radiotherapy where insertion or application of radiation sources is done directly into or on the surface of a tumor.
- • This is an essential component of the treatment of gynecological cancers e.g. cervical and endometrial cancer.
- • This technique is also used in the treatment of cancer of the tongue and cheek.
- • In addition, the radiation source can be placed in the tumor bed post operatively such as soft tissue sarcomas, for a short, intense course of radiotherapy.
- • The High Dose Rate (HDR),Iridium192 source ensures a short treatment time e.g. in minutes rather than days as in the case of low dose rate radiotherapy (LDR).

With the help of this machine we can perform Image Guided Brachytherapy (Also MRI based Brachytherapy) of all types e.g. Intra-cavitary, Interstitial, Intraluminal, Endo-bronchial and Surface Brachytherapy.
